General Information for Dental Services
Single persons, 1 through 20 years of age, who do not have Medicaid (Title XIX), CHIP, or an
WIC Voucher, need to go through a dental eligibility process. This can be done at La Nueva
Casa de Amigos, 1809 N. Main Room 134, 713-547-8010, Monday through Friday. For dental
eligibility at Lyons, Northside, and Sunnyside Health Centers, see an Eligibility Screener. Upon
determination of one’s eligibility, a City Dental Card and a “Notice of Eligibility and Copay” form
will be issued. Please take the card and form in person to the Dental Clinic of your choice to
schedule an appointment.
Persons, 1 through 20 years of age, who have Medicaid (Title XIX) coverage, can make
appointments by taking the current Medicaid card (Form #3087) to the Dental Clinic of their
choice. Persons, 1 through 18 years of age, who have CHIP coverage, do not initially need a
City Dental card. They can make appointments by taking the CHIP card to the Dental Clinic of
their choice. The WIC enrolled children can make their appointments by presenting a current
WIC voucher to the dental clinic of their choice.
All pregnant women, enrolled in the City of Houston Maternity Clinics, are eligible for one dental
visit without a City Dental card. If additional services are necessary, a City Dental card and a
“Notice of Eligibility and Copay” form are required.

Las Personas solteras, de 1 a 20 años de edad, que no tengan Medicaid (Título XIX), CHIP, o
cupones de WIC, necesitan pasar por el proceso de elegibilidad dental. Hay elegibilidad dental
en La Nueva Casa de Amigos, 1809 N. Main Oficina 134, 713-547-8010, de Lunes a Viernes.
Para la elegibilidad en las Clínicas de Salud de Lyons, Northside, y Sunnyside, hay una
persona encargada de la elegibilidad. Al determinar su elegilibilidad, se le dará una Tarjeta
Dental de la Ciudad y una “Nota de Elegibilidad y Forma de Pago.” Favor de llevar está tarjeta
en persona a la Clínica Dental de su elección y solicitar una cita.
Las personas, de 1 a 20 años de edad, que tengan cobertura de Medicaid (Título XIX) pueden
hacer su cita al traer su tarjeta más reciente de Medicaid (Forma #3087) a la Clínica Dental de
su elección. Las personas, de 1 a 18 años de edad, que tengan cobertura de CHIP
inicialmente no necesitan de la Tarjeta Dental de la Ciudad. Ellos pueden hacer su cita al traer
su tarjeta de CHIP a la Clínica Dental de su elección. Los niños registrados en el programa
WIC podrán hacer sus citas presentando un cupón WIC vigente, a la clínica dental de su
elección.
Todas las mujeres embarazadas, registradas en las Clínicas de Maternidad de la Ciudad de
Houston, son elegilbles a una visita dental sin la Tarjeta Dental de la Ciudad. Si se requieren
servicios adicionales, es necesario tener una Tarjeta Dental de la Ciudad y una “Nota de
Elegibilidad y Pago.”
