Any child entering Kindergarten in August, 2013 who has NOT participated in a VPK program.

June 10th - August 8th

Your child’s birth certificate
3 proofs of residency (copy of utility bill, lease agreement, etc.)
Immunizations or shot records form (Blue form)
Most recent physical within the last year (Yellow form)
Please note:
The VPK program has an attendance policy
LUNCH and BREAKFAST will be provided
Parents are responsible for their own transportation

Favor de Tomar Nota:
Los programas VPK incluyen una póliza de asistencia, la cual los padres deben
firmar y estar de acuerdo en observar.
SE SERVIRA DESAYUNO Y ALMUERZO
Los padres son responsables de proveer transportación al Programa
