Eligibility Requirements To qualify for CHIP, your child must: • Be under 19 years of age • Be a citizen of the United States (U.S.), a U.S. National, or lawfully residing as determined by the United States Citizenship and Immigration Service (USCIS) • Be a Pennsylvania resident • Not be enrolled in a private insurance program AND • Not be eligible for or enrolled in Medical Assistance through the Department of Public Welfare Requisitos de elegibilidad A fin de calificar para CHIP, su hijo: • Debe ser menor de 19 años • Ser ciudadano de los Estados Unidos (EEUU), un nacional de Estados Unidos, o residan legalmente según lo determinado por la ciudadanía de Estados Unidos y el servicio de inmigración (USCIS) • Debe ser residente de Pennsylvania • No debe estar inscrito en un programa privado de seguro • No debe ser elegible ni estar inscrito en el programa Medical Assistance a través del Departamento de Bienestar Público (Department of Public Welfare) IND373 4/15 © Blue CRoss of Northeastern Pennsylv ania. 2015.
