MENINGITIS
VACCINE
Please be advised that all
Firecats entering their
senior year (12th grade)
for the 2016-2017 school
year will need the following immunization: 2
doses of Meningococcal
vaccine.
If your student has already received this
immunization please
submit an updated shot
record to the main office by Monday, May 2nd,
2016.
Beginning Tuesday, May
3rd, 2016 all students
must submit proof of
immunization being
administered or submit
an appointment card.
Failure to submit either
documentation will
result in a lasalle for
everyday that they are
non-compliant.

ABSENCE & DR NOTES
Remember To Get Your
Receipt For Your Attendance Excuse Notes!
Submit all excuse notes
directly to the main
office staff at the front
counter. Students have
2 days upon return to
school to submit a note
and avoid demerits. All
students will receive a
receipt to serve as proof
of successful submission
of the required excuse
note. Students should
save this receipt in case
of any error in submission. All notes should include the students first &
last name, grade, date of
absence & a parent signature. If you have any
questions please contact
the main office.

Por favor tenga en cuenta que todos los Firecats
que estarán entrando
en su último año de
secundaria (12 grado)
