Winnings will be awarded from donations received. Winner need not be present to win. Buy 10
tickets and receive 11th ticket free. All winners will be announced in the parish bulletin and must
fill out a W-9 form to receive winnings. Once St. Agnes receives this order form, we will email/mail
your receipt to you and we will place your Raffle Ticket in the raffle barrel.

Premios serán otorgados de las donaciones. No tienes que estar presente para ganar. Compra 10
tiquetes/boletos y recibe el 11avo tiquete/boleto gratis. Todos los ganadores serán anunciados en el
boletín parroquial y tendrán que llenar un formulario W-9 para recibir el premio. Una vez que la Iglesia de
St. Agnes reciba este formulario con tu donación te mandaremos por email/correo tu recibo y pondremos
tu tiquete en el barril de rifa.
