Rationale of Sponsor Eligibility: Sponsors for the Sacrament of “Baptism and/or Confirmation must be Catholics who
have been confirmed and have already received the Sacrament of the Most Holy Eucharist and who lead a life in harmony
with the faith and role to be undertaken. (Code of Canon Law, CC874 and 893)
I HEREBY AFFIRM my Faith in the Lord Jesus Christ and my fidelity to the teachings of the Roman Catholic Church;
I AFFIRM that I have received the Sacraments of Baptism, Holy Eucharist and Confirmation;
I AFFIRM that I practice my Catholic faith by regularly joining in the celebration of Sunday Mass and Communion;
That, if married, my marriage has been celebrated in the Catholic Church
That, if I have children, they are being education in the Catholic Faith;

con su fe y posición que están por tomar. (Código de la Ley Canónica, CC 874 y 893)
