ITEMS TO BRING TO YOUR CLINIC APPOINTMENT
• A biological parent or legal guardian must attend the initial
appointment with a minor child and present a driver’s license
or other official photo ID. Very limited exceptions apply. Legal guardians must also bring a copy of the guardianship
documents. Please call the clinic as soon as possible if you
have questions about this requirement. • Complete list of medications
• The patient’s current health plan card(s)
• Medical records as requested by the schedulers. Please
bring copies of x-rays and/or CT/MRI studies. A digital copy
on disc is preferred. • Payment of co-pay or co-insurance fees can be made
with Visa, MasterCard, Discover, cash or personal check. Payment is necessary at the time of service, if required by
your health plan(s).
• New patient forms: For your convenience, we have included
most of the forms that a new patient in the practice is
required to complete. Please either complete these in
advance or arrive at the clinic at least 30 minutes prior to your
appointment time and we will be happy to assist you. • Free public access WiFi is available in the lobby for personal
digital devices.

ART ÍCULOS PARA TRAER A SU CITA
• Un padre biológico o tutor legal debe asistir a la cita inicial
con un niño menor de edad y presentar una licencia de
conducir o otra identificación oficial con fotografía. Se aplican
excepciones muy limitadas. Los tutores legales también
deben traer una copia de los documentos de tutela. Por
favor, llame a la clínica lo mas pronto posible si usted tiene
preguntas acerca de este requisito. • Tarjeta vigente de el plan de salud del paciente
• El pago de su co-pay o cargos que cobra su seguro puede
ser pagado con tarjeta de Visa, MasterCard, Discover,
efectivo, o cheque personal. Es necesario pagar el mismo
día que recibe el servicio si así lo requiere su plan de salud.
• Lista completa de medicamentos
• Formas para pacientes nuevos: Para su comodidad, hemos
incluido la mayoría de las formas de que se requieren de
completar para un paciente nuevo. Por favor, complete estos
antes de llegar a la clínica o por lo menos llegue 30 minutos
antes de su cita y estaremos encantados de ayudarle.
• Registros médicos que la recepcionista pidió cuando hizo su
cita. Favor de traer copias de radiografías y / o estudios de
las tomografías computarizadas y resonancias magnéticas.
Se prefiere una copia digital en disco.
• Acceso gratis de Wifi es disponible en el lobby para los
dispositivos digitales personales.
