Healthcare on the Horizon:
The Patient-Centered Medical Home
Going to see a doctor can sometimes be
confusing and frustrating. At a doctor’s
office, a patient generally has little time
to spend with the doctor. Also, usually
only one issue or problem is addressed
at the appointment. This experience
can make a patient feel rushed and
unimportant. The appointment may
also leave a patient with more questions about their illness, treatment, and
follow-up care. However, having a Patient-Centered Medical Home (PCMH)
can change this experience.

A Patient-Centered Medical Home is
a team that plans and responds to the
needs of the patient. Everyone on that
team will understand the problems of
the patient1. A PCMH has the following

will have a doctor who they will have
a continuous relationship with2.
Physician-Directed Medical Practice - A team will be developed to
respond to the needs of a patient.
This team will be led by the patient’s
personal physician2.
Whole-Person Orientation - The personal physician will work together
with the patient through their health
concerns and provide referrals if they
need to see other doctors2.
Integrated Care - There will be open

Quality and Safety - The team will constantly be monitored to improve and
ensure safety and high-quality care2, 3.
Access - Each patient will be able to
see their personal physician at more

Payment - Certified Patient-Centered
Medical Homes will receive compensation for improving the quality of care
for their patients1, 2.
What are the benefits of a
PCMH?
There are many benefits to having a
PCMH. First, a patient will have more
time with their doctor. In a PCMH, a
doctor will give tasks to other team members. This will give the doctor more time
to spend with patients, address the needs
of the patients, and educate them about
treatment options3, 4. If a patient needs care
from a specialist, then the doctor will coor-

dinate the care with a specialist. Also, a patient in a PCMH
will receive more reminders about upcoming appointments4. Finally, healthcare costs are predicted to be lower
for those in a PCMH even with the extra costs of these
services. A study of a PCMH in North Carolina found that
it saved Medicaid $60 million in 2003 and $120 million in
20041. Another study concluded that every $1 investment
in making a PCMH will save/earn a clinic $1.503. This savings may result in lower costs for patients in a PCMH.
Promotores(as) de Salud and the PCMH
In 2010, President Barack Obama signed the Patient
Protection and Affordable Care Act (PPACA) into law.
This law recognizes the value of Patient-Centered Medical
Homes and provides funding for expanding its use5. The
PPACA also provides funding for increasing the number of
Promotores(as), or Community Health Workers (CHWs), as
government officials understand that CHWs can be valuable
to PCMHs6.
Promotores(as) can play an important role in PCMHs
because they both share similar goals like whole-person
orientation, care integration, and quality and safety assurance. Promotores(as) are members of the community that
PCMHs are trying to serve, so they understand the community and its needs. They can tell doctors about the problems
facing their communities and help them to develop solutions. Another benefit of Promotores(as) is that they can
help patients understand how the U.S. health system works.
Finally, Promotores(as) can improve the care given to patients by teaching them about prevention, screening, and
chronic disease management7. For example, Promotores(as)
can teach patients about the importance of getting routine
vaccines for children, having mammograms, and exercising
and dieting for patients with diabetes8.

Tener que ir al doctor a veces puede ser confuso y frustrante. En la oficina del médico, un paciente generalmente

Paciente?
Un Hogar Médico Orientado al Paciente es un equipo
médico de personas que planea y responde a las necesidades
del paciente. Todas las personas en el equipo entenderán
los problemas del paciente1. Un PCMH tiene las siguientes

cual tendrá una relación continua2.

del paciente. Este equipo será guiado por el médico
personal del paciente2.

trabajará en conjunto con el paciente para ayudarle con
sus preocupaciones de la salud y proveerá referencias si
el paciente necesita visitar a otros médicos2.
Cuidado Integral - Habrá comunicación abierta entre
el médico personal, otros médicos (como especialistas)
y/o la comunidad que rodea al paciente2, 3.
Calidad y Seguridad - El equipo será constantemente
monitoreado para mejorar y asegurar la seguridad y
buena calidad del cuidado2, 3.
Acceso - Cada paciente podrá ver a su médico personal
durante horas más convenientes2, 3.
Pago - Un Hogar Médico Orientado al Paciente certificado recibirá compensación por mejorar la calidad del
cuidado para sus pacientes1, 2.
¿Cuáles son los beneficios de un PCMH?
Hay muchos beneficios en tener un PCMH. Primero, un
paciente tendrá más tiempo con su doctor. En un PCMH,
un doctor les dará tareas a otros miembros del equipo. Esto
le dará más tiempo para pasar con sus pacientes, abordar
sus necesidades y educarlos sobre las opciones de tratamiento3, 4. Si un paciente necesita cuidado de un especialista, el doctor coordinará este cuidado con el especialista.
También, un paciente en un PCMH recibirá más recordato-

rios sobre sus citas pendientes4. Finalmente, con un PCMH
se pronostica que los costos del cuidado de salud son más
bajos para los
pacientes, aún
con los gastos
adicionales de
estos servicios.
Un estudio
mostró que en
un PCMH de
North Carolina Medicaid
ahorró $60 millones en 2003 y
$120 millones
en 20041. Otro
estudio determinó que por
cada $1 de inversión en crear un PCMH, una clínica ahorrará/ganará $1.503. Estos ahorros pueden resultar en costos
más bajos para pacientes de este sistema.

importancia de recibir vacunas rutinarias para los niños, de
realizarse mamografías y, para los pacientes con diabetes, de
hacer ejercicio y seguir una dieta8.

A dinámica is an activity or game with a specific purpose, such as
to: get to know one another, learn new information, review what
was learned, brainstorm new ideas or just get a group moving.
Una dinámica es una actividad o un juego con un propósito
específico, por ejemplo: conocerse uno con otro, aprender nueva
información, repasar información aprendida, inspirar nuevas
ideas o para que el grupo se mueva.
Dinámica
Stuck Like Glue
Purpose: To show the importance of teamwork and
communication.
Group Size: 10 to 30 people
Materials: Tape or rope
Procedure:
1. Make a start line and finish line about four or five feet apart with tape or rope.
2. Each team member must have his or her feet “glued” to the person next to him or her.
3. The goal is to reach the finish line without becoming unconnected from each other. If
anyone on the team becomes unconnected, the whole team has to start over.
Discussion:
1. How was communication within the team?
2. Were all the ideas heard? Why or why not?
3. How did the team achieve its goal?
4. What qualities from this activity can apply to a PCMH?
Possible Answer: In a PCMH, each person contributes to the team. A doctor may know more about the patient’s medical
history; a nurse may have more knowledge about the patient in general; and a Promotor(a) may have a personal relationship
with the patient and his/her family and have a better understanding of the patient’s social environment. Everyone must come
together and share their “expertise” with the team in order to come up with the best solution, like in this activity.

4. ¿Cuáles son las cualidades de esta actividad que se puedan aplicar a un PCMH?
Posible respuesta: En un PCMH, cada persona contribuye al equipo. Un doctor puede saber más de la historia médica de su
paciente; una enferma puede tener más conocimiento sobre el paciente en general; y un Promotor(a) puede tener una relación
personal con el paciente y su familia y entender mejor el ambiente social del paciente. Todos tienen que juntarse y compartir su
“conocimiento”con el equipo para poder determinar cuál es la mejor solución, tal como en esta actividad.

experience.

experiencia.

Anteriormente, Asistente de Salud Adolescente del programa
de Infórmate
I participated in the Infórmate (Teen Health Aide) program
at DuRussels’ Potato Farm in Manchester, Michigan in 1999
and also in Texas during my senior year of high school. In
Michigan, I was
a Teen Health
Aide and in the
Texas program I
participated with
a theater troupe
that taught
teenagers about
the prevention
of HIV. It was
a rewarding
experience to
inform my peers
of health issues.
I was educated in
various topics from sexually transmitted diseases to pesticide exposure, and then I was able to educate other farmworkers.
The MHP coordinators always talked about the importance
of a college education. In one of the MHP activities, a
group from the Michigan State University CAMP (College
Assistance Migrant Program) visited and talked about their
scholarship opportunities. I went on to be accepted to MSU
and was able to graduate in 4 years with a degree in Agricultural Business Management.
MHP provided a great foundation for my life. I will forever
be grateful for the opportunities and experiences that MHP
provided. The coordinators, Esmeralda Perez-Rawlings
in Texas and Delissa Garcia and Maria Helena Abreu in
Michigan, were great; they were always helpful and always
provided great advice. MHP has made a positive and lasting impact on many migrant teenagers that are sometimes
lost and forgotten. In my case, they opened that first door
that eventually took me to MSU to earn a degree.
Currently, I work with U.S. Department of Agriculture –
Rural Development as a Multi-Family Housing Specialist.
As for my family, I have a daughter, Isabella Sophia, who
will be 4 years old in October, and my son, Sebastian, was
born in June.
Participé en el programa de Infórmate (Asistente de Salud
Adolescente) en DuRussels', una granja de papa en Manchester, Michigan, en el año 1999 y también en Texas durante mi
último año de secundaria. En Michigan, yo era un Ayudante
de Salud Adolescente y en el programa de Texas participé con
un grupo de teatro que educó a los adolescentes sobre la prevención del VIH. Fue una experiencia gratificante de informar a mis compañeros de los temas de salud. Me eduqué en
diversos temas desde enfermedades transmitidas sexualmente
hasta la exposición a los pesticidas, y así fue como pude educar a otros trabajadores agrícolas.
Los coordinadores de MHP siempre hablaban de la importancia de la educación universitaria. En una de las actividades
de MHP, un grupo de la Universidad Estatal de Michigan
(MSU) del programa de CAMP (Asistencia Universitaria
para Migrantes) vino a visitarnos y nos habló sobre sus oportunidades de becas. Fui aceptado en MSU y me pude graduar
en 4 años con una licenciatura en Administración de Empresas Agropecuarias.
MHP proporcionó una gran base para mi vida. Siempre
estaré agradecido por las oportunidades y experiencias que
MHP me dio. Las coordinadoras, Esmeralda Perez-Rawlings
en Texas y Delissa Garcia y Maria Helena Abreu en Michigan, eran grandiosas; siempre fueron amables y siempre
dieron buenos consejos. MHP ha marcado un impacto
positivo y duradero en muchos adolescentes migrantes que a
veces están perdidos y olvidados. En mi caso, abrieron esa
primera puerta que eventualmente me llevó a MSU a obtener
un título.
Actualmente, trabajo con el Departamento de Agricultura de
los Estados Unidos – Desarrollo Rural como Especialista en
Vivienda Multifamiliar. En cuanto a mi familia, tengo una
hija, Isabella Sophia, quien cumplirá 4 años en octubre, y mi
hijo, Sebastian, nació en junio.
