Instructions:
Before you participate in your Sacramental Celebration or Parish Spiritual Event discuss what is a
sacrament? Need clarification? Look in the Catechism of the Catholic Church. See Part Two, Section Two “The Seven
Sacraments of the Church.”
Review and answer the following family discussion questions:
Name and briefly describe the Sacrament or Spiritual Event you participated in:
What traditions does your family have surrounding this Sacramental or Spiritual Event Celebration?
How do those traditions help you to see and experience the presence of God in the Celebration? Where do you find God?
After attending the celebration, do the following activity suggested for each child wishing to receive credit:


Grade K - 2: Draw a picture about the sacrament on a sheet and attach to this form. Parents please write a
caption or explanation of the picture.
Grade 3-6: Answer these questions. What did you learn from the celebration of this sacrament or event? How
did or will this celebration help you better understand and live your faith?
Attach and turn in to Faith Formation Office to get class time credit by April 1, 2016.

discuta que es un Sacramento. Necesita clarificación. Busque en el Catecismo de la Iglesia Católica. Lea la Segunda Parte,
Segunda Sección: “Los Siete Sacramentos de la Iglesia.”
Lea y conteste como familia las siguientes preguntas:
Nombre y describa brevemente el Sacramento o evento en el que participo:
¿Qué tradiciones tiene su familia relacionada con este evento o celebración sacramental?
¿Cómo les ayudan esas tradiciones a ver y vivir la presencia de Dios en esta celebración? ¿Dónde encontró a Dios?
Después de asistir a la celebración, haga las siguientes actividades sugeridas según el nivel de cada niño que desee recibir
crédito:


Kínder-2do Grado: Hagan un dibujo que represente el proyecto realizado y adjúntelo a esta planilla. Padres por
favor escriba una descripción explicando lo que el dibujo representa.
3er Grado-6to Grado: Contesten las siguientes preguntas en una hoja aparte: ¿Qué aprendiste de este evento o
celebración sacramental? ¿Cómo te ayudara esta celebración a entender mejor y vivir tu fe?
Devuelva esta hoja a la Oficina de Formación de Fe para obtener crédito de una clase antes del 1º de abril, 2016.
