These tools were developed by the Fistula First Catheter Last (FFCL) Workgroup Coalition and focus on planning for a vascular access, fistula maturation, and graft healing. The "Vascular Access Planning Guide for Renal Professionals" is a comprehensive guide with all the steps in planning for and placing an arteriovenous (AV) fistula or an AV graft. It is a companion resource to Lifeline for a Lifetime: Planning for Your Vascular Access, the access planning guide for use in educating patients. The "Ready Set Go" tools were designed to aid renal professionals in understanding the stages of fistula maturation and graft healing and to help them educate patients with companion patient resources. The Lifeline for a Lifetime complete vascular access planning and monitoring toolkits for patients and renal professionals include: Easy access to downloadable PDFs for patients and professionals Videos that can be downloaded and saved for continued use Many patient resources are available in English and Spanish. Now Available for Download Page 2 NetLink January 2016 2016 QIP Quality Improvement Project This year, 2016, will see the addition of a quality improvement activity centered around the QIP. The base year of the new contract will work on the topic of Hypercalcemia performance. ESRD Network 14 will select 10 or more facilities in Texas that achieved the poorest performance on the Hypercalcemia measure, and these facilities will work with the Network to build performance in this area. Previously, selected facilities were generally enrolled in a project for the entire year. This project, however, allows the facility to exit the project once success is quantified. At that time, facilities that have met the improvement target of at least 25% relative improvement from baseline or exceed the ESRD QIP penalty threshold will be able to exit the project, and ESRD Network 14 will select facilities to replace those that have shown improvement. 2016 Patient Engagement Calendar The Patient Engagement Calendar will not be updated for 2016. The CROWNWeb Data Management Guidelines are intended to streamline the data validation process and increase the accuracy of CROWNWeb data. The CROWNWeb Data Management Guidelines must be followed by all Medicare-certified dialysis facilities, whether or not they are part of a batch submitting organization (e.g., DaVita, FMC, DCI, and NRAA). Things to know: Tier 1 refers to facility-level CROWNWeb users. Deadlines for certain CROWNWeb data validation exercises have been changed. These guidelines are intended for all Medicare-certified dialysis units. The guidelines are divided into three phases: Monitoring, Measuring, and Managing. All tasks that facility-level CROWNWeb users are responsible for executing are listed in the tables. The CROWNWeb Data Management Guidelines can be downloaded from www.mycrownweb.org or from the ESRD Network 14 CROWNWeb page at http://esrdnetwork.org/professionals/qip/ crownweb/. The Network strongly encourages all facilities to download these guidelines and train all facility-level CROWNWeb users on the new requirements and deadlines. The ESRD QIP is no different. Performance year 2016/Payment year 2018 features the most robust version of the ESRD QIP yet. As we begin 2016, we will focus on the newest measures each month. The first measure we will focus on has a data collection period that began in October 2015. PATIENTS: Are you interested in helping the Network understand how patients feel about different subjects? Would you like to be a voice for other patients? STAFF: Do you have engaged patients and families and/or active Facility Patient Representatives (FPRs) that would like to speak up on behalf of your other patients? Be an SME! What do patient SMEs, family members, and caregivers do? Provide your voice as part of a larger whole, the entire ESRD patient population in the state of Texas. • Represent patients of all treatment modalities. • Share ideas to help improve patient-centered care and communication. • Review patient education materials to ensure they are helpful for patients. • Teach your fellow patients, providers and other stakeholders. • Learn from your fellow patients, providers, and other stakeholders. • Celebrate the success of your efforts. What projects do SMEs work on? Here are some examples: Patient Advisory Committee (PAC): Advise the Network on issues that affect all the dialysis and transplant patients in Texas. Patient Grievances: Help improve communication between patients and staff to reduce conflict. Patient Experience of Care (ICH CAHPS): Improve the quality and experience of care across the state. Vascular Access: Help increase fistulas and decrease catheters to improve patient health. Infection Prevention: Contribute ideas on ways to reduce infections, prevent sepsis, and keep patients healthy. Vaccinations: Share your ideas on ways to encourage patients to accept recommended vaccinations. Home Dialysis: Help the Network understand how to educate patients on home therapy programs. Emergency Preparedness: Help patients learn how to prepare for and respond to an emergency.
