PHYSICAL EXAMINATION REQUIREMENTS
At the time of initial enrollment or registration, students entering a Florida school for the first time must provide a valid physical
examination.
 The physical must have been completed in the U.S.
 The date of the physical exam must be within 12 months of the child’s first day of school.
 Students who cannot provide proof of a valid physical examination may not attend school until they can provide the
necessary information required on the Florida form DH 3040.
 An appointment card from a local physician or clinic showing that a physical examination is scheduled to be completed at a
later date is not acceptable.
 Physical examinations can be obtained from your physician, health care provider, or a walk-in clinic before registering your
child. A list of clinics and pediatricians is available at your child’s school or by calling 377-0514 or 377-0110.

The Florida Department of Health – Collier County provides school age immunizations, free of charge, on a walk-in basis at the following
sites in Naples and Immokalee. Clinics in Naples and Immokalee are closed at 12 Noon on the last working day of each month.

RECONOCIMIENTO MEDICO REQUERIDO
En el momento de la matrícula inicial a una escuela de la Florida por primera vez los estudiantes deben presentar un reconocimiento
médico “válido”.
 Realizado en los Estados Unidos.
 Dentro de los últimos doce meses antes del primer día de clases del estudiante.
 Los estudiantes que no puedan presentar una prueba válida de Reconocimiento Médico, serán excluidos hasta que ellos
puedan proporcionar la información necesaria requerida en el formulario DH 3040 de la Florida.
 No será aceptada ninguna tarjeta de una clínica o de un doctor particular la cual muestre una cita donde se llevará a cabo
dicho reconocimiento médico.
 Se puede obtener el reconocimiento médico de su doctor o clínica antes de matricular a su hijo. Una lista de clínicas y
pediatras está disponible en la escuela o llamando al número 377-0514 o 377-0110.

las siguientes localidades en Naples é Immokalee. Las Clínicas en Naples e Immokalee estarán cerradas a las 12:00 del mediodía en el último
día de trabajo de cada mes.
