After picking up the Baptismal Package from our office, you must try to fulfill the
requirements (first step is to know when and where parents and godparents are taking the
baptismal preparation class ! if taking class at our parish, you must RSVP/Limited
space). Please be aware that you baptismal date and time are not official until you submit,
at least two weeks prior your desired date, this complete package with all its requirements
(incomplete packages will not be admitted):
! Submit: Sacrament of Baptism Registration Form” (other side). Carefully read “Baptismal Guidelines and Requirements”
" Submit: Child’s Birth Certificate. Parents and legal guardians can only register a child for baptism.
# Submit: Proof of baptismal preparation class attendance from Parents and Godparents.
$ Submit: Letter of Candidacy. Both sponsors must give you the completed forms signed and stamped by their priest.
! Submit: Donation of $ if registered or not. Cash or check accepted.
Note: Before submitting your package, please confirm the date and time of baptism; dates and times may change.

Después de recoger su Paquete Bautismal de nuestras oficinas, deberá buscar reunir los
requisitos (iniciar por buscar dónde y cuándo van a tomar las clases pre-bautismales los
papás y padrinos ! para pláticas en nuestra parroquia deberá reservar su lugar). Favor de
tener en cuenta que la fecha y hora del bautismo no es oficial hasta que todo el paquete y
requisitos hayan sido entregados, por lo menos entregar dos semanas antes del bautismo,
en nuestras oficinas (no se aceptan paquetes incompletos):
! Entregar: “Forma de Registro Sacramento del Bautismo” (reverso). Leer bien la “Guía y Requisitos para el Bautismo”
" Entregar: Acta de Nacimiento del infante. Solamente los padres o tutores legales del infante pueden hacer trámite.

DIVINE SAVIOUR CHURCH & ST. ANN CHURCH: SACRAMENT OF BAPTISM GUIDELINES & REQUIREMENTS W
elcome! The pastoral staff of our parishes of the Divine Saviour and St. Ann shares with you the joy of the
soon celebration of your child’s Sacrament of Baptism. The following guidelines are to assist you in
preparation for your child’s baptism, and are to be observed so that as a community we may enjoy a prayerful
celebration that reflects the Christian values we hold.
1. Baptismal arrangements must be completed in person by the parents or legal guardians only. Date and time of
baptism are not official until the completed application and all requirements are submitted to our office at least
two weeks prior to the date.
2. The child must be six (6) years old or under to follow the regular procedure of receiving the Sacrament of
Baptism. If the child is seven (7) years old or older, he/she must be incorporated to our Religious Education
Program to be prepared for the Sacraments of Baptism and First Communion. Additional Information is
available at our office.
3. Requirements for the Sacrament of Baptism are:
− The Registration for Sacrament of Baptism Form must be filled out and signed by a parent or guardian.
− The birth certificate of the child must be presented with the completed Registration for Sacrament Baptismal
Form. We will take the legal name of the child as it is on the birth certificate. We cannot change, add or
modify names.
− You are to select Godparents who are practicing Catholics, attend mass regularly and receive communion;
who have received the sacraments of Baptism, First Holy Communion and Confirmation and at least 16 year
of age. They must be either single or married in the Catholic Church. Only one Godfather and/or one
Godmother can be a baptismal sponsor. The sponsors must turn in the “Letter of candidacy”.
− Parents and godparents must attend a Baptismal Preparation Class here or at any other parish that offers
baptismal preparation classes. Those choosing to attend class at Divine Saviour must be enrolled for a class
($10 per person donation and must register before the class day. No same day registration) and
will receive a certificate of completion to submit with the application. Those attending class elsewhere must
submit proof of class attendance at the time the completed application is submitted. Classes are offered in
Spanish & English at Divine Saviour Church on the first Wednesday of the month at 7:00 p.m. (Convent).
− The donation is $40.00 per child for registered and not registered parishioners. If the family cancels or does
not show for the ceremony there is no reimbursement and a new application (date) requires a new donation.
4. Parents and godparents must be on time for baptismal preparation class (parents and godparents only-no children
please) and on the actual day of the Baptismal Ceremony (parents, child and godparents). If parents, godparents or
both are late, we reserve the right to postpone the baptism; we will give you options about when your child may
receive the sacrament in a later date (requires new registration-donation). All are asked to plan on arriving at the
preparation classes and to the church at least twenty (20) minutes prior to the class and/or ceremony.
5. Instruct your friends and relatives, including godparents, to arrive at the church at least twenty (20) minutes
prior to the ceremony. As the baptism ceremony will take place in the church, which is the house of God, we
ask that the dress code on the behalf of the parents and godparents be modest upscale casual to formal.
Women’s dresses must be of a modest cut (no bare shoulders, no strapless dresses and no cleavage) and no mini
skirts or short pants are allowed. Shawls are permitted. Males must wear upscale casual-formal dress attire. No
jeans, saggy pants, no sandals, etc. We ask you to let your family know how special this moment is in your life
and in the life of your child and ask them to dress accordingly.
6. No eating, chewing gum or drinking is allowed in the church. Silence and reverence must be shown at all times
in the church.
7. If any of the requirements are not met, we reserve the right to cancel or postpone the sacrament.

IGLESIA DEL DIVINO SALVADOR E IGLESIA DE SANTA ANA: GUÍA Y REQUISITOS PARA EL BAUTISMO ¡
Bienvenidos! Mientras usted y su familia se preparan a celebrar el Sacramento del Bautismo, el equipo pastoral de
las parroquias del Divino Salvador y Santa Ana comparten ese gozo del bautismo de su niño. A continuación le
presentamos la siguiente información para asistirle en la digna preparación en la celebración del Sacramento del
Bautismo. Los siguientes lineamientos deberán de ser observados para que como comunidad celebremos y
disfrutemos una ceremonia que refleje nuestros valores Cristianos que profesamos:
1. Los arreglos para el Sacramento del Bautismo deberán ser hechos por los padres o los guardianes (tutores)
oficiales del niño(a). La fecha y la hora del bautismo no son oficiales hasta que la aplicación y los requisitos son
completados y entregados en nuestras oficinas a lo menos dos semanas antes del día del bautismo.
2. El niño(a) deberá de tener seis (6) o menos años de edad para participar en la preparación ordinaria a recibir el
Sacramento de Bautismo. Si el niño(a) tiene más de siete (7) años de edad deberá ser incorporado al programa
de educación religiosa para prepararse de una forma más completa a recibir el Bautismo y la Primera Comunión.
3. Requisitos para el Sacramento de Bautismo:
− Favor de presentar la aplicación Registration for Sacrament of Baptism Form (Registro para el Bautismo)
completa y firmada por un padre de familia o guardián del niño(a).
− Al momento de registrar al niño(a) para el sacramento, se deberán presentar el acta de nacimiento del
niño(a). El nombre del niño(a) aparecerá de la misma forma que en el acta de nacimiento. No podemos
cambiar ó añadir nombres que no concuerden con el acta de nacimiento.
− Los padrinos que vayan a escoger deberán haber recibido los Sacramentos de Bautismo, Primera Comunión
y Confirmación. Además, deben profesar la fe de la Iglesia Católica; asistir a misa los domingos y comulgar;
tener por lo menos 16 años; ser soltero/soltera o si los padrinos son casados, deberán de ser casados por la
Iglesia Católica. Solamente puede haber un Padrino y/o Madrina por bautizado. Los padrinos deberán
entregar la “Carta de Candidatura” junto con la aplicación de bautismo.

− Los padres y padrinos deberán asistir a las Clases Pre-bautismales en la parroquia del Divino Salvador
(Convento) o en otras parroquias (ver parroquias abajo) y presentar los comprobantes de su asistencia con la
aplicación. Hay que registrarse antes y hay una donación de $10 por persona; no se permite registrarse la
misma noche de la plática.
− La donación es de $40.00 por niño/niña si la familia (los padres) está registrada o no en nuestra parroquia. Si
cancela ó no se presentan para la ceremonia no hay reembolso de dinero y para registrarlo de nuevo requiere
un nuevo proceso que también incluye otra donación.
Los papás y padrinos deberán estar siempre a tiempo para las Pláticas Pre-bautismales (Padres y padrinos
solamente, no niños en las pláticas por favor) y en el día del bautismo (padres, niño y padrinos). Si los padres
con el niño y los padrinos no están a tiempo para iniciar el bautismo, nos reservamos el derecho de posponer el
bautismo para otro día, requiere nueva registración-donación. Favor de llegar a tiempo.
Favor de hacer saber a sus familiares y amigos (incluyendo a los padrinos) el estar en la iglesia por lo menos
veinte (20) minutos antes de la ceremonia. Padres, favor de asegurarse que la forma de vestir de ustedes y
padrinos es digna, modesta y formal. Porque la iglesia es la casa de Dios, las mujeres deberán de asegurarse que
sus vestidos no estén escotados por enfrente y por la espalda (no enseñen los hombros, evitar vestidos strapless,
etc.), no minifaldas ni shorts. Se permiten tipos de rebozos que cubran las partes escotadas durante la
ceremonia. Los hombres deberán también de venir bien vestidos (no mezclilla rota, pantalones demasiados
sueltos, ni shorts, ni sandalias). Les pedimos que también les recuerden a sus familias que tan especial es este día
para su niño y para ustedes, por favor pídanles que vengan bien vestidos.
No comida, bebidas ni masticar chicle es permitido en la iglesia. Silencio y reverencia deberá ser observado
siempre en la iglesia.
Si alguno de estos requisitos no es completado nos reservamos el derecho de cancelar o posponer el bautismo.
Clases pre-bautismales son a las 7:00 p.m. el 1º Miércoles de cada mes (favor de registrarse antes de asistir)

The role of the Godparents is “together with the parents, to present an infant at baptism, and help the baptized to lead a Christian life in
harmony with baptism, and to fulfill the obligations connected with it.” (Canon 872). Thus, the Church has established the following
norms governing the qualifications needed to be a Godparent. The Godparent must be: At least sixteen (16) years old; “A
Catholic who has been confirmed and has already received the Sacrament of the Holy Eucharist and leads a life in harmony
with the faith and role to be undertaken…Not be bound by any canonical penalty…Not be the father or mother of the one
to be baptized” (Canon, 874, Sección 1). Only one Godfather and/or one Godmother can be baptismal sponsor (Canon
873), but additional Catholic persons act as witnesses at the baptism. I addition, “a baptized person who belongs to a noncatholic ecclesial community may not be admitted except as a witness to baptism and together with a Catholic Sponsor.”
(Canon 87, Section 2). A non-Christian may not be a Godparent or witnesses.

El papel de los padrinos es “junto a los padres, presentar un infante en el bautismo y ayudar al bautizado a llevar una vida cristiana en
armonía con el bautismo y cumplir fielmente con las obligaciones conectadas a él.” (Cn 872) De este modo, la Iglesia ha establecido los
siguientes requisitos necesarios para ser un(a) padrino/madrina. El padrino (madrina) deberá: Tener no menos de dieciséis
(16) años; Ser “un(a) Católico(a) que haya sido cofirmado(a) y ya haya recibido el Sacramento de la Sagrada Eucaristía y que lleve una vida
en armonía con la fe y el papel que va a emprender…No estar confinado por ninguna pena canónica…No ser ni el padre ni la madre de quien
va a ser bautizado.” (Cn, 874, Sección 1). Solamente puede haber un Padrino y/o Madrina por bautizado (Cn 873), pero
personas Católicas adicionales pueden actuar como testigos en el bautismo. Además, “Una persona bautizada que pertenece a
una comunidad eclesiástica no católica solamente puede ser admitida como testigo en el bautismo si es acompañada por un padrino/madrina
Católico(a)” (Cn 87, Sección 2). Una persona que no es Cristiana no puede ser ni padrino/madrina o testigo.
